The first stage of any relationship is establishing communication. The following health surveys are designed to provide Dr. Kincaid with the initial data he needs to evaluate your case. At the same time, this will establish a doctor/patient relationship as required by the state of Texas for Dr. Kincaid to work with you. We will acknowledge receipt of your survey via e-mail and then telephone you when it has been evaluated. There is a $15.00 charge for the initial evaluation and report, which will be credited against your first order. Do not email your credit card number. For security reasons we prefer to handle that over the telephone. After the initial report, a full consultation with Dr. Kincaid will be scheduled to discuss your situation as presented in both the survey and your (faxed) medical history.
